The purpose of this amendment is to answer questions that were received (see below) Q. What were the actual annual trip volumes for the past three years? A. Approx 2023= 8,900 Actual 2024=9,622 Actual 2025=10,200 Total 28,722 over 3 years Q. Can you provide a breakdown of average monthly volume to help assess potential seasonal fluctuations? Average 26 per week January April  Average 29 per week May -Aug Average 32 per week Sept-Dec Q. What are the peak days of the week and peak hours for trip demand?

A. Monday, Wednesday, Friday 7:00am 3:00pm (Dialysis Patients) Q. What percentage of transports are same day requests versus those scheduled in advance? A. Most are pre-scheduled. Approximately 3% requested on the same day Q. Are bariatric transports anticipated under this requirement, and if so, approximately what percentage of total trips? A. 20-25% of our population are bariatric patients.

Q. Beyond invoicing requirements, is there a required recurring performance reporting cadence such as weekly or monthly operational reports? A. The CO/COR will typically have weekly meetings initially then can push out to monthly meetings to review/discuss performance/operations. Q. Can you provide the data below?
