Notice of Intent to Sole Source Requirement The Department of Veterans Affairs, NCO 10 Contracting Activity intends to negotiate a sole source contract under Simplified Acquisition Procedures and the authority of FAR Part 6.302-1, Only One Responsible Source for the following items listed below for the Detroit VA Medical Center.

The associated NAICS code for this is 325412. The vendor will be CuraScript Infusion Pharmacy (small business) 11403 Bluegrass PKWY, Suite 400, Louisville, KY 40299. Description of requirements: Photofrin Medication 75mg 58914-0155-75 CuraScript is the manufacturer and distributor for Photofrin. This is an Emergency Request; the patient s procedure is scheduled for this 02/14/2025. Delays in placing this order will directly impact the quality and timeliness of patient care.

This action will be Sole Sourced to CuraScript Infusion Pharmacy, a Small Business entity, IAW FAR Part 13.106-1(b)(1)(i) A firm fixed price PO with no options is anticipated and shall be awarded. It is the Government's belief that only the proposed candidate possesses the required capabilities to successfully meet this requirement. It is the Government's intention to solicit and negotiate with only this source. This notice of intent is not a request for competitive proposals.
