INTENT to Sole Source - Roche Accu-Chek VISN 10

Notice of Intent to Sole Source Requirement Department of Veterans Affairs, VISN 10 Healthcare System Network serving Ohio, Indiana, and Michigan, intends to award a sole source, firm fixed price BPA (Blanket Purchase Agreement) contract under FAR (FAR 8.405-6). Only One Responsible Source for the brand name proprietary software interface and diabetes test strips and supplies listed below, as they are only available through the original equipment manufacturer.

The associated NAICS code for this is 334516 for Lab Supplies and small business size standard of 1,000 employees. The vendor selected is Roche Diagnostics Corp., 9115 Hague Road, Indianapolis, IN 46250 Description of Equipment or Supplies: RALS Plus Accu-Chek Inform II Meter Kit, Accessory Box Kit, & Supplies A firm fixed price contract with four option years is anticipated and shall be awarded.

It is the Government's belief that only the proposed candidate possesses the required capabilities to successfully meet this requirement. It is the Government's intention to solicit and negotiate with only this source. The BPA will provide for continued usage of equipment currently on-site at VISN 10 Medical Centers and Community Based Outpatient Clinics. VA personnel utilize the equipment and supplies to perform patient WBG (whole blood glucose) testing.
