STATEMENT OF WORK VA Northern Indiana Health Care System (VANIHCS)- Ft Wayne, IN Project:
610A4-21-201 Build Sim Lab Project Background: Building 1 is pre-1950 construction; Asbestos Containing Material (ACM) and lead based paint are present in the construction zone and must be removed and handled per OSHA guidelines. The contractor is responsible to have a 3rd party contractor identify all hazardous materials.
Project Overview: The project converts room 36 in the basement of building 1 into a Simulation Lab at the VANIHCS-Ft Wayne campus to accommodate the education department to better serve the nursing staff.
This will be accomplished by removing an HVAC system from an adjacent mechanical space and creating various training rooms inside of room 36. Scope of Work: The contractor must provide all supervision, labor, material, and equipment required to completely renovate/modify room 36 and an adjacent mechanical space at the VANIHCS-Ft Wayne campus into a simulation lab. The total square footage is approximately 1,500 SF.
The simulation lab will consist of a classroom, a control room, a mock patient room, and a mock exam room. The classroom will have glass markerboards, smart monitors, and workstations. The mock patient room will have a working head wall, shower, toilet, and sink. Next, the control room will have one-way windows into both the mock patient room and the mock exam room. Finally, there will be storage for mannequins and other training equipment and supplies.
ACM and lead based paint are expected to be present in the construction area. All hazardous material must be removed and handled per OSHA guidelines. The contractor shall conduct their own ACM testing to confirm the location & type. As a working Hospital the adjacent spaces and functions will need to be maintained and a high degree of coordination and scheduling will be required to allow the continuation of the existing Hospital Services during the entire Construction Project.
All shutdowns will require a two-week minimum coordination period. Surrounding areas in the basement will remain active and there will be additional construction in the basement for other projects. Follow construction drawings and specifications provide by the A/E firm. All ACM and demolition debris weights shall be recorded and reported to the VA Contracting Officer Representative (COR). Testing and Balancing (TAB) shall be included.
See Annex A for standardized name brand or equivalent items to be included in your bid. All work to be completed at the VA Northern Indiana Health Care Ft Wayne Campus, 2121 Lake Ave, Ft Wayne, IN 46805. Work to be completed within 210 calendar days from notice to proceed. Justification for this Project: The existing space in the Annex is not conducive as a simulation lab.
The area is just a plane room, it must be set up before any training, it is not co-located at the main hospital, it does not simulate an exam room or patient room, and it is too small. This project will gut the entire room 36 and remodel/renovate the space to today s standards allowing the VA to better serve its nursing staff.
