**NOTICE OF INTENT TO SOLE SOURCE** This is a Notice of Intent to Sole Source published in accordance with Federal Acquisition Regulation (FAR) 5.101(a)(1) requiring the dissemination of information regarding proposed contract actions.

This Notice of Intent is for a proposed award of an open market sole source award for Mizuho Orthopedic Systems, Inc., Complete Maintenance Services to support the Department of Veterans Affairs, Hershel "Woody" Williams VA Medical Center, 1540 Spring Valley Dr, Huntington, WV 25704. The proposed award is to, Mizuho Orthopedic Systems, Inc (SAM UEI: ZX7ZPD78JUA1), the sole manufacturer, sales, and service provider of the Mizuho OSI operating room tables.

This includes all software, hardware, training, and service components. Mizuho OSI requires anyone working on the tables to be certified by and with Mizuho OSI. These are specialty surgical tables and require software upgrades that cannot be carried out by anyone but their team. Their services include travel, labor, and parts with unlimited technical phone support. Award will be made under the authority of FAR 13.106-1(b); Soliciting from a single source. The Period of Performance is for one base year (12 months).

The North American Industry Classification System (NAICS) code for this service is 811210 (Electronic and Precision Equipment Repair and Maintenance). Product Service Code (PSC) is J065 (Maintenance, Repair and Rebuilding of Equipment: Medical, Dental, and Veterinary Equipment and Supplies). Small business size standard is $34.0 million.
