SUMMARY: Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office 5, is issuing this request for information/sources sought notice in order to identify vendors that are interested in and capable of providing contractor services for cable and internet services. For the Huntington VA Medical center.

CABLE AND INTERNET SERVICES Location: Hershel Woody Williams VAMC Huntington VA Medical Center 1540 Spring Valley Rd Huntington, WV 25704 Description of Work: The contractor shall provide cable and internet services to main facility, mental health clinic, RRTP, and Fisher house. Services to the Main Campus to include:
109 rooms of Bulk Television, 1 gb Dedicated Internet Access (DIA) x 1each, brand name or equal to Zoom Pro 300 internet services, 9 High-Definition Digital Adapters, and associated fees. Services to the Mental Health Clinic to include:
brand name or equal to Zoom Pro 100 internet services, and fees. Services to the Building 9, Residential Rehab Treatment Program (RRTP) to include:
Television services, 3 High-Definition Digital Adapters, and associated fees. Services to the Fisher House to include:
Television Service, 17 High-Definition Digital Adapters, and associated fees.
Background: Cable and Internet services are used at the facility for our patients. Provide service to equipment on site as need (to include boxes, cables, and remote controls).
Performance Period: It is anticipated the service is required for the next 60 months. Base period with four years option years. 10/1/2024-9/30/2025 10/1/2025-9/30/2026 10/1/2026-9/30/2027 10/1/2027-9/30/2028 10/1/2028-9/30/2029 B. GENERAL REQUIREMENTS All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms.
C. SPECIAL CONSIDERATIONS C.3.1 Contractor Furnished Materials All labor, materials, equipment, and supervision to perform all aspects of the contract. When furnish , provide , install , or similar term is used it shall mean a complete installation, ready for use. The work will include all items as described in the Scope of Work listed above. C.3.2 Government Furnished Materials and Services.
Government furnished property, if any, will be identified and will only be used for the performance of this contract unless otherwise authorized by the Contracting Officer. The Contractor shall, always, take any and all steps necessary for maintenance and preservation of all Government-furnished property. The Contractor shall comply with all reasonable requests of the Contracting Officer to enclose, or specifically protect, Government-furnished property.
C.3.3 Qualifications of Key Personnel Contractor s employees should be knowledgeable of standards associated with this type of work. Interested & Capable Responses:Â NCO 5 is seeking responses from entities that are interested in this procurement and consider themselves to have the resources, capabilities, and qualifications necessary to provide the services required for the Huntington VA Medical Center.
Please respond to this RFI/Sources Sought 36C24524Q0579 with your: Business name (including Unique Entity ID (SAM.gov)  Business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), and  Person of contact (including telephone number & email address).   Capability statement and summary of previous experience providing these types of services for the Veterans Health Administration or other similar facilities.  Available contract vehicles (GSA FSS, etc.) for use by Department of VA (include applicable NAICS) Brief Summary of categories of services that your company can/cannot provide. Certification(s) / Licenses for meeting the local requirements to provide these services at the DC VAMC (if applicable) Confirmation that your organization has the capability & expertise to meet the performance requirements while complying with applicable limitations in subcontracting by responding to the questions below with your response:   Do you intend to self-perform this effort? If you are located outside the immediate area, how will you self-perform? Or do you intend to subcontract work under this contract? If you intend to subcontract work under this contract, how will you ensure compliance with the limitations on subcontracting?  Response Instructions: Responses must be submitted by 10:00 AM (ET) July 10th, 2024.
