NOTICE OF INTENT TO SOLE SOURCE The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 5), Martinsburg VA Medical Center located at 510 Butler Avenue, Martinsburg WV, 25405 is issuing this Notice of Intent (NOI) to inform industry contractors of the Government s intent to execute a Sole Source award IAW FAR 13.106-1(b)(1) and only one responsible source and no other supplies or services will satisfy agency requirements per 41 USC §3304(a)(1) as implemented by FAR 6.302-1.

This is for Integrated Power Console, Bundle IPC SYS ENDOSCRUB 2 W/CART, product # IPCES2SYSKIT2 and StraightShot M5 Microdebrider, product # 1899200 for Martinsburg VA Medical Center. Standardization of the equipment is vital to ensure standardization of supplies. Medtronic is the Original Equipment Manufacturer (OEM) and the sole manufacturer and sole supplier of the requested items as mentioned above. These products are only available through Medtronic representatives.

The place of performance is Martinsburg VAMC, 510 Butler Avenue, Martinsburg WV, 25405. This action will result in a Firm-Fixed Price Contract. The intended Contractor is Medtronic USA, Inc. located at 6743 Southpoint Dr N Jacksonville, FL 32216. The Small Business Administration (SBA) size standard for this sector is 1,000. This notice of intent is neither a formal solicitation nor a request for competitive proposals. No solicitation document is available and telephone requests will not be honored. No award will be made based on unsolicited quotations or offers received in response to this notice. Any response to this notice must show clear, compelling, and convincing evidence that competition will be advantageous to the Government.
