This is a combined synopsis/solicitation for items prepared in accordance with FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-24-Q-0123. This procurement is 100% Indian Owned set-aside under NAICS code 237110 with a size standard in millions of dollars $45.0.

The Contractor shall furnish and install a sewer system and lift station as shown on the drawing and the statement of work. The period of performance for this requirement is 60 calendar days from Notice to Proceed. The official site visit 11:00 AM June 4, 2024 at 1908 E 10th Street Okmulgee, OK 74447. Questions are due no later than J June 11, 2024 5:00 p.m. CST. The closing date for receipt of quotes is June 24, 2024 COB CST.

The offer shall be received by email to jeffrey.earnest@ihs.gov or in a sealed envelope, addressed to this office, showing the solicitation number, your name and address, no later than 5:00 p.m. CST, on June 24, 2024. This synopsis/solicitation is anticipated to be under $50,000.

THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING: A) Firm, Fixed price quote with attached bid sheet B) Technically specifications of the statement of work, acceptable materials list to be used. C) (2) Relevant past performance of this type of work, shall provide organization’s name, point of contact, summary of project and award amount D) Bid bond form (SF24) attached is required at $35,000.

If over $35,000 and selected as the acceptable vendor proposal, performance (SF25) and payment (SF25A) bonds shall apply Offers shall be evaluated on an acceptable/non-acceptable basis. Unacceptability of "A" and "B" (stated above) shall render the entire quote unacceptable. Evaluation shall proceed as follows:

1. Quotes shall be ranked by price, from low to high 2. Quotes shall then be evaluated for technical acceptability beginning with the lowest priced quote 3. This process shall proceed until a technical acceptable quote is identified THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS IN THE STATEMENT OF WORK CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR OFFERING THE LOWEST REASONABLE PRICE AND MEETING ALL TECHNICAL SPECIFICATIONS.

The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions.
