THIS IS A NOTICE OF THE GOVERNMENT’S INTENT TO ISSUE A SOLE SOURCE AWARD CONTRACT TO SOUTHERN PROSTHETIC SUPPLY, INC FOR PROSTHETIC AND ORTHOTICS SUPPLIES The Defense Health Agency (DHA) intends to award a sole source blanket purchase agreement under the statutory authority, FAR 6.302-1(a)(2)(i)(A) Only one responsible source and no other supplies or services will satisfy agency requirements. DHA intends to issue a firm-fixed blanket purchase agreement to Southern Prosthetic Supply, Inc, 6530 Corporate CT Alpharetta, GA 30005-8003 (Unique Entity ID:

JA85UAFKBXH2, CAGE Code: 6Y0E9). The Government partnered with Southern Prosthetic Supply, Inc to procure orthotic and prosthetic supplies and devices. Devices are made to the exacting specifications and requirements of individual patients to restore physiological function and/or cosmesis. This requires access to a large catalog of product options and materials in order for practitioners to develop fit and function based on each patient’s needs. The anticipated period of performance will be for five (5) years.
