THIS IS A NOTICE OF THE GOVERNMENT’S INTENT TO ISSUE A SOLE SOURCE AWARD CONTRACT TO MILE HIGH ORTHOTICS LABORATORY FOR PROSTHETIC AND ORTHOTICS SUPPLIES The Defense Health Agency (DHA) intends to award a sole source blanket purchase agreement under the statutory authority, FAR 6.302-1(a)(2)(i)(A) Only one responsible source and no other supplies or services will satisfy agency requirements. DHA intends to issue a firm-fixed blanket purchase agreement to Mile High Orthotics Laboratory, 4970 Monaco Street Unit A Commerce City, CO 80022-4624, (Unique Entity ID:

D7VDYL1JZH23, CAGE Code: 5EZ86). The Government partnered with Mile High Orthotics to procure orthotic and prosthetic supplies and devices. Devices are made to the exacting specifications and requirements of individual patients to restore physiological function and/or cosmesis. This requires access to a large catalog of product options and materials in order for practitioners to develop fit and function based on each patient’s needs.

Laboratory Contractor will provide 3D Printed orthotics braces, custom footwear and more. The custom 3D printed Orthotics can be customized with various top cover materials. By printing the orthotics enables adding a post directly to the shell making it stronger and more accurate to the mm. 3D printed orthotics are sustainable and a good transition for companies looking to use environmentally friendly products.

Custom shoes are carefully crafted to meet the unique needs of patients to reduce pain and maximize health benefits. The anticipated period of performance will be for five (5) years.
