June 21, 2024 THIS IS A NOTICE OF THE GOVERNMENT’S INTENT TO ISSUE A SOLE SOURCE AWARD CONTRACT TO CASCADE DAFO, INC FOR PROSTHETIC AND ORTHOTICS SUPPLIES The Defense Health Agency (DHA) intends to award a sole source blanket purchase agreement under the statutory authority, FAR FAR 6.302-1 (2) Only one responsible source and no other supplies or services will satisfy agency requirements. DHA intends to issue a firm-fixed blanket purchase agreement to Cascade Dafo, Inc 1360 Sunset Ave Ferndale, WA 98248-8913 (Unique Entity ID:

PDHCZ75AQBF3, CAGE Code: 6Y0E9). The Government partnered with Cascade Dafo, Inc to procure orthotic and prosthetic supplies and devices. Devices are made to the exacting specifications and requirements of individual patients to restore physiological function and/or cosmesis. This requires access to a large catalog of product options and materials in order for practitioners to develop fit and function based on each patient’s needs.

To provide an orthotic design that is integral in the treatment of dependents with Cerebral Palsy, Down Syndrome, Spina Bifida, and similar diagnoses. Additionally, the design is utilized for our Wounded Warriors with Traumatic Brain Injuries, Limb Salvage, and other neurological diagnoses. The anticipated period of performance will be for five (5) years. The primary North American Industry Classification System (NAICS) code is 339112 – Surgical and Medical Instrument Manufacturing.
