## Description

There is no solicitation at this time. This request for capability inforamtion does not constitute a request for proposals; submission of any information in response to this market survey is purely voluntary; the government assumes no financial responsibility for any cost incurred.

**Requirement:** The Non-Clinical Nurse Case Managers ensure that every West Virginia Army National Guard (WVARNG) soldier is medically ready to execute his/her state and federal mission. This is achieved by identifying those Soldiers not meeting medical deployment standards, tracking their care through deployability determination or medical separation from the force, and maintaining accurate documentation throughout the process.

The Non-Clinical Case Managers work with the State Surgeon and other members of the WVARNG Medical Detachment, the G1 Human Resources department, and Unit Command teams to ensure proper medical case management of all personnel assigned to the WVARNG. Obtain completed release of medical information forms from Soldiers as required and submit these forms to the Soldiers’ medical provider to obtain all pertinent documentation. Coordinate Soldier treatment plans with appropriate military and civilian medical facilities. Educate Soldiers on resources available to improve medical and dental readiness.
