## Description

The Medical Readiness Contracting Office – Pacific intends to issue a within scope modification to contract W81K0222P0015. The reference contract was awarded as an Individual Service Provider contract on a competitive basis to Chan W. Park, LLC under the authority of the simplified procedures for certain commercial items. The services that are being performed are General Surgeon (Bariatric) at Tripler Army Medical Center, Hawaii. This modification will affect Option III.

The period of performance of 01 January 2025 to 31 December 2025 remains unchanged. The anticipated date for contract modification is on or about 10 July 2025. The purpose of this modification is to increase quantities for General Surgeon (Bariatric) services for Option III to support the increasing needs of the Department of Surgery at Tripler Army Medical Center, Hawaii. This announcement fulfills the synopsis requirements under FAR 5.201 and 5.203.
