## Description

The Indian Health Service, Navajo Area, Shiprock Service Unit located at U.S. Highway 491 North, Shiprock, NM 87420 intends to negotiate a single source award with Milestone Medical Technologies, Inc. (UEI: X9A2U2JNF257), for a twelve (12) month/yearly service contract plan to cover an UltraSAFE Formulin Dispensing system. The projected period of performance is a total of five (5) years from the award date.

**Base Period:** twelve (12) months from time of award. Option year-one: approximately from 6/01/2026 to 5/31/2027 Option year-two: approximately from 6/01/2027 to 5/31/2028 Option year-three: approximately form 6/01/2028 to 5/31/2029 Option year-four: approximately from 6/01/2029 to 5/31/2030 The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 811210 – Electronic and Precision Equipment Repair and Maintenance with a small business size of $34.0 million.

In accordance with FAR 13.106-1(b)(1)(i), for purchases not exceeding the Simplified Acquisition Threshold, Contracting Officers may solicit from one source if the Contracting Officer determines that the circumstances of the contract action deem only one source reasonably available. This Notice of Intent to Single Source is not a request for quotations and a solicitation will not be issued in conjunction with this Notice of Intent.

Any vendor that believes it can provide the stated service contract may provide a capability statement. Verbal responses are not acceptable and will not be considered. Responses to this notice shall be submitted no later than 2:00 PM (MDT) on May 12, 2025 via electronic mail to Dallas Begay, Purchasing Agent at Dallas.Begay@ihs.gov. A determination by the Government on whether to compete this proposed requirement based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competition or proceed with a single-source award.
