## Description

**Amendment 001:** This amendment adds the IEE Representation Form as an attachment. **Original Notice:** This sources sought is in support of Market Research being conducted by the Phoenix Area Indian Health Service to identify potential ISBEE sources capable of providing comprehensive Preventative Maintenance & Repair Services for Sterilization Equipment at Phoenix Indian Medical Center (PIMC).

The support required will involve technical, labor, tools, equipment, literature, and necessary travel to provide maintenance/calibration verification and onsite repair services as recommended by the original equipment manufacturer (OEM).

The contractor shall provide all services, personnel, expertise, supplies and incidental materials necessary to execute the performance requirements detailed in the Statement of Work (SOW). *THIS IS A SOURCES SOUGHT ONLY - DO NOT SEND PRICING INFORMATION OR PROPOSALS IN RESPONSE TO THIS NOTICE*
