## Description

The Contractor shall provide -Clinical Data on Residents of Long-Term Care Facilities Supporting CDC Programs and Emergency Response- as follows:

1) Comprehensive record-level patient and encounter information, medication administration information, and facility characteristics with weights;

2) Data summaries and supporting material to allow for CDC personnel to quickly utilize the comprehensive data source;

3) High-level data extract summaries with each data delivery that allow for comparison of key metrics for the updated data transmission;

4) Technical support and professional services to support ad hoc projects and help with customized data extraction and analysis.
