This U.S. industry comprises establishments of business entities that can transfer patients on behalf of Indian Health Services between four (4) geographical locations. These services include the transfer of medically acute patients that require a higher level of inpatient medical or behavioral health services. The contemplated award designee is University of Arizona doing business as the Arizona Poison and Drug Information Center through the previously established AZ REACH program, located in Tuscon, Arizona. This will be for Indian Health Services, Phoenix Area Office, 40 N Central Phoenix Arizona 85004, for a period of five (5) years, under Federal Acquisition Regulation (FAR) 6.302-1 Only one responsible source and no other supplies or services will satisfy agency requirements.
