## Description

**Acquisition Description:** This is a Notice of Intent, not a request for proposal. The National Institute of Health, Clinical Center (NIH-CC) intends to negotiate on an other than full and open competition basis with Telecor Inc. The NIH's Department of Clinical Research Informatics (DCRI) is responsible for the network infrastructure that supports the technology of all mainframe and desktop computers, and the management and development of applications and databases.

The NIH Clinical Center (CC) provides complex inpatient and outpatient care and supports extensive biomedical research. The hospital’s Point-of-Care (POC) program enables immediate diagnostic testing using bedside analyzers that connect to the hospital’s Laboratory Information System (LIS) and Electronic Health Record (EHR). The Clinical Center relies on the Telcor QML® middleware system to manage all POC device interfaces, capturing and transmitting patient test results securely to Soft LIS and CRIS.

Annual renewal of software support and interface licenses from Telcor, Inc. is required to maintain uninterrupted operation and compliance with CAP, CLIA, and NIH IT security standards.

**Justification:** The NIH Clinical Center uses Telcor QML® as the central middleware system for all point-of-care testing interfaces. The QML software integrates hundreds of decentralized testing instruments with NIH’s clinical systems. Because QML® is proprietary and licensed exclusively by Telcor, Inc., only the OEM can provide authorized software maintenance, updates, and technical support.

Replacing this system would require redevelopment of the entire middleware architecture, extensive revalidation, and unacceptable downtime of critical patient care systems. Other middleware vendors including Orchard Trellis, Data Innovations, and Siemens RAPIDComm provide systems that are not compatible with the Telcor QML middleware environment currently being used. Telcor, Inc. is currently a direct seller and they do not have resellers.
