PSC CODE: R497 “Support-Professional: Personal Services Contracts” NAICS CODE: 624120 “Services for the Elderly and Persons with Disabilities” REQUIRED RESPONSES Interested companies should include the following information in their responses: (1) CAGE Code; (2) Name and address of firm; (3) Size of business including average annual revenue for past three years, number of employees, and type of small business (8(a), Women-Owned, HUB-Zone, Veteran-Owned, Service-Disabled-Veteran-Owned, etc.); (4) Country of ownership; (5) System for Award Management (SAM) information; (6) DUNS number, if available; (7) Number of years in business; (8) Point of contact and alternate to include name, title, phone, and email address; (9) A clear, concise statement of the business’s capability in meeting the requirement; and (10) Small businesses shall address how they will comply with FAR 52.219-14: Limitations of subcontracting There is no specific format or outline submittals must follow.
