## Description

**IMPORTANT UPDATE NOTE:** This requirement estimates 6 users on a regular basis utilizing the Injury Tracking Program Subscription. The software solution for this requirement must be available via Azure Government. This requirement is for a computer-based Injury Tracking Program Subscription that will help with implementing a program to input medical information to improve communication and allow tracking of students.

It is intended to track the most common injuries and collect data to utilize best practices for student safety and to decrease injuries in training environments. The Injury Tracking Program Subscription will help optimize communication with leadership and class teams regarding student status, participation levels, and medical appointments. Data will also be used to monitor injury trends and identify events causing the most injuries.

Additionally, the information will be compared to Physical Readiness Test (PRT) performance to evaluate any correlation between performance and increased injury risk. The program leverages HIPAA-approved application (app) to allow immediate communication without requiring computers at all locations. The program will be utilized via apps on phones, computers, and kiosks for data entry.

As a result of this solicitation, the Government intends to award a Firm-Fixed Price (FFP) Contract with One (1) Base Year and Four (4) Option Years. See attached solicitation document - 4.02 RFQ N0018925QR132 - for additional details.
