## Description

THIS IS A SOURCES SOUGHT NOTICE This solicitation is a “SOURCES SOUGHT” Only. This is not a request for formal proposals or quotes. No formal solicitation document exists at this time. Your response to this announcement will be treated as information only. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of contractor submission of responses to this announcement or the Government us of such information.

This request does not constitute a solicitation for proposals or the authority to enter into negotiations to award a contract. No funds have been authorized, appropriated, or received for this effort. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. The intended contract period will be for one year to start at time of award.

**Purpose and Objectives:** The proposed contractor shall provide clinical laboratory patient testing, when requested by staff physicians, that are not done at the Wagner IHS Healthcare Facility. These tests are typically of the communicable type and by having the test ran at the State Department of Health, turnaround times to report and track are decreased. This allows for the possible decrease in transmissions of these diseases.

The intent of this Sources Sought notice is to identify potential offerors capable of Response: Responses to this Sources Sought Notice should be able to demonstrate the firm’s ability, capability, and responsibility to deliver the services listed above.

Responses should include the following information: Business name, address, business size, Socioeconomic Category will be all Small Business concerns (Indian Economic Enterprise Small Business, HUBZONE, 8a, Service Disabled Veteran-Owned Small Business, Veteran-Owned Small Business, Small Business, etc.), UEI number, and point of contact to include e-mail address and telephone number.

All offerors must be registered in the (www.sam.gov), and Representations and Certifications must be completed prior to any future awards. Any offeror capable of providing this service shall notify the Purchasing Agent by e-mail by February 3 at 12:00 p.m. CST. The NAICS Code is 541380 – Testing Laboratories and Services. All information is to be submitted via e-mail to john.archambeau@ihs.gov. Determination by the Government not to compete this proposed contract action based on responses to this notice is solely within the discretion of the Government.
