## Description

**THIS IS A BUY INDIAN SET ASIDE per Buy Indian Act, 25 U.S.C. 47 and only offers form Buy Indian small businesses will be accepted. ** The Indian Health Service, Shiprock Service Unit, Northern Navajo Medical Center, located at US Hwy 491 North, Shiprock, New Mexico 87420 has a requirement to one (1) Operating Room Assistant (Supply Technician). Services will be performed in accordance to the attached Statement of Work. The Period of Performance will base period for 6 months with an option to extend and additional 6 months.

**Base:** 03/03/2025 to 09/2/2025, Option to extend: 09/03/2025 to 03/02/2026 The North American Industry Classification System or NAICS code is 561320 – Temporary Help Services. Quotes will be due February 20, 2025 at 11:00 am. Return the attached price schedule – rates are all inclusive. Reference notice number in the subject line. The principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be used.

Evaluation factors may include technical capability to meet the government need outlined in the statement of work, past performance, and price. Any multiple offers will not be forwarded for evaluation and further consideration. Quotes will be due on February 20, 2025 no later. In accordance with 52.204-7(b)(1) prospective contractors must have – and maintain (in accordance with 52.204-13(c) – an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.
