## Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included I this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Refer to enclosed Combined Synopsis-Solicitation_IHS1502379 PMA ULTRASOUND.

The Indian Health Service (IHS), Chinle Comprehensive Health Care Facility, located in/at Off Highway 191 & Hospital Drive, Chinle, AZ 86503 needs a contractor to provide On-Site Preventative Maintenance for Radiology Ultrasound Equipment. Services shall be provided in accordance to attached Scope of Work.
