NOTICE TO CONTRACTOR This is a combined synopsis solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12. This announcement constitutes the only solicitation. Offers are being requested, and a separate written solicitation will not be issued. Solicitation number HT941026Q2015 is issued as a Request for Quote (RFQ). Defense Health Agency Contracting Activity (DHACA) Healthcare Contracting Division-West (HCD-W) requests responses from qualified sources capable of providing the following:
