## Description

Defense Health Agency (DHA) has a requirement for a Firm Fixed Price (FFP) IDIQ supply contract in accordance with FAR Part 12, Commercial Items and FAR Part 13, Simplified Acquisition Procedures for Pre-Filled Syringes and Bags for the Pharmacy Department at Naval Medical Center Portsmouth. In order to be considered for this requirement, please respond with a completed proposal following the guidelines in the Instructions to Offerors (page 45) and Evaluation Factors For Award (page 48) of the SF1449.

The amended solicitation HT940625QE012 0001 corrects Attachment 2- Past Performance Questionnaire where the title of the requirement was incorrect and it has been corrected to IDIQ Pre-filled Syringes and Bags at the Naval Medical Center Portsmouth.
