## Description

The 75th Air Base Wing Chapel requires family retreat services. The services will be performed within a 45-minute drive of Hill Air Force Base Utah 84056 in accordance with the Performance Work Statement (PWS). The PWS is attached for the description of the requirement. The government intends to establish a single award Blanket Purchase Agreement (BPA) with a three year ordering period of 15 April 2025 to 14 April 2028. The government prepared the following documents to aide in the preparation and submission of the offeror's quote.

1. FA820125Q0011 Combo Synopsis Solicitation. The combo document contains the 52.212-1 Instructions to offerors and 52.212-2 Evaluation.

2. PWS 14 Feb 2025 3. Wage Determination 2015-5483 Rev 25 4. Clauses SF 1449. The documents contains clauses and is required to be used for the offeror to submit proposed prices for CLINs 0001 to 0007. 5.
