## Description

**UPDATE:** 26 March 2025. Attached PDF FA521525R0004 Kokee Asbestos Testing QAs. This identifies what questions were received and provides the Government's response. Drawings and floor plans are included in this attachment. Please see attached RFQ document and its attachments in addition to this description. This is a Request for Quote. We are seeking eligible vendors to provide quotes for the below:

The Contractor will undertake a comprehensive assessment and sampling procedure, which includes ACM (Asbestos-Containing Materials) testing for buildings 1, 5, 10, and 14100, with a total area of approximately 20,000 square feet. Adequate sampling will be conducted on the floors, walls, pipes, ceilings to ensure the government receives thorough and accurate information for each facility.

Contractor will ensure adequate testing is done in the upstairs support section room, downstairs radar tech offices, kitchen/common area, and the bedrooms of building 1 and the results for these areas should be specifically denoted in the report. See attached for a Map of the buildings. The below reports shall be submitted to the government within 15 business days of the contractor having acess to the installation. These reports are:

**Asbestos Inspection Report:** A detailed report of the inspection findings, including the locations and conditions of asbestos-containing materials (ACMs).

**Laboratory Analysis Reports:** Results from bulk sampling and laboratory analysis, including Polarized Light Microscopy (PLM) analysis.

**Asbestos Abatement Plan:** If abatement is required, a plan outlining the procedures and safety measures to be followed.

**Chain of Custody Forms:** Records of the handling and transportation of asbestos samples.

**Updated Asbestos Management Plan:** Revised plan reflecting the current status of ACMs in the building. Email is the preferred means of communication for all responses to this solicitation. Please provide any questions to the POCs by 12:00 PM (HST) 25 March 2025.
