## Description

**Premier Shockwave, (CAGE:** 57X35) 823 Herron Road, Knoxville, TN 37932 is the sole owner of the OssaTron® medical device(s) that uses Extracorporeal Shock Wave Treatment (ESWT), to address chronic musculoskeletal conditions. A master blanket purchase agreement (BPA) shall be entered into with Premier Shockwave, to provide address chronic musculoskeletal conditions with their OssaTron® medical device(s).
