**Vbrick OnPrem Subscription Maintenance Period of Performance:** 10/15/2026 - 10/14/2027 (base) 10/15/2027-10/14/2028 (option period 1) 10/15/2028 -10/14/2029 Rev Private - Named User Subscription (Renewal or Add-On Only) - Video platform for managing and accessing Live and On-Demand video for Private Cloud and On Prem. Includes user and admin portal and reporting. Priced per user per year. Includes support.

TECHNICAL EVENT SERVICES PREMIUM SUBSCRIPTION - Ann. subscription includes pre-event testing, day-of support and post event reporting. Pricing is per event (min.1). Unused events expire each subscription year end. Remote only. T & E not included. Please complete and submit the following information: Pricing Schedule, Section B - Supplies or Services and Prices/Costs of the solicitation, including a time of delivery or delivery schedule as well as a technical point of contact. (Please complete and return the attached version) Software License, Maintenance, Support Agreements for Government Customers.

Pre-Award Supply Chain Risk Management Form (attached) All requested information shall be submitted to Christina V. Brooks via email at chrbrooks@fdic.gov, referencing the solicitation number and including the name of your company and address.
