This announcement constitutes the only solicitation; proposals are being requested. The solicitation number is 86615625R00001 and the solicitation is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2025 -02. This requirement is unrestricted. This tool will be available on a subscription for up to 125 users. The contractor shall provide access to a Web-based portal dashboard report system, including customizable dashboard homepage risk rating report featuring summarized data encompassing regulatory surveys, occupancy, re-hospitalization, enforcement actions, abuse/neglect issues, candidacy for special focus, likelihood of adverse outcomes, and automated alert notifications for up to 5,000 facilities.
