## Description

The Indian Health Service, Tsaile Health Center, located in Tsaile, AZ 86556 off of Navajo Route 64 &12 has an upcoming requirement for a contractor to provide Family Physician services. Period of Performance is, 12 months duration beginning from date of award, base period plus two one-year option periods.

**Base Period:** 1 year First Option Period: 1 year Second Option Period: 1 year The North American Industry Classification System or NAICS code is 561320 with a Size standard in millions of dollars, $34 million. The Request for Quotations No. 75H71025Q00286 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 8/11/2025.
