SUMMARY OF SCOPE The project is for the installation of supporting utilities for the new Philips X-Ray unit at the Fort Thompson IHS Clinic. The new machines require different support structure, electrical, and a change in flooring layout. In accordance with FAR 36.204, Disclosure of the Magnitude of Construction Projects, the anticipated project magnitude is between $100,000 and $250,000. The type of work to be performed will be categorized under PSC code Y1DA, Construction of Hospitals and Infirmaries and NAICS code 236220, Commercial and Institutional Building Construction with a size standard of $45M. Bond Capacity: Information on the firm's bonding capability - specifically identify single and aggregate bonding capacities. Experience Submission Requirements: Submit at least two (2) but no more than five (5) projects completed by your firm within the last six (6) years that are similar to the work that will be required under this project. Firms shall include the following information for each project: Indicate whether Prime Contractor or Subcontractor for each project submitted; Dates of construction for each project submitted; Contract value, location, completion date, and complexity of job for each project submitted; Indicate whether the project is a federal, state, tribal or other for each project submitted; and Project references/Agency point of contact (telephone number and email address) for each project submitted. If the entity responding to this sources sought is submitting experience information from any entity that does not match the name, CAGE code, and/or UEI exactly as stated on the submitting entity’s SAM record (e.g. affiliates, subsidiaries, parent companies, sister companies, etc.) the submission shall clearly demonstrate how that entity will have meaningful involvement in the performance of the contract and specifically address which resources would be committed to the contract. Failure to comply with this requirement or failure to adequately demonstrate meaningful involvement and commitment of resources on behalf of that entity may result in that experience information to be considered not relevant. Along with the project experience submission, the following questions shall be answered in your firms’ submission: How is your company structured to handle quality control of the various disciplines of a construction project? End of Amendment A0001 ######################################################################################################## This is a Solicitation Notice for solicitation number 75H70125Q00019, Fort Thompson Health Center X-Ray Replacement Project, a construction project for the Indian Health Service (IHS), Division of Engineering Services (DES) prepared in accordance with FAR Part 36. The purpose of this project is for the installation of supporting utilities for the new Philips X-Ray unit at the Fort Thompson IHS Clinic. The X-Ray unit was already purchased by the service unit and it is not included in this project. The radiology unit at the Fort Thompson IHS Clinic currently has an old GE X-Ray unit. These units will be replaced by a new Philips X-Ray machine. The new machines require different support structure, electrical, and a change in flooring layout. This project will be procured as a 100% Small Business set-aside solicitation under North American Industry Classification System (NAICS) 236220, Commercial and Institutional Building Construction. The associated size standard for this procurement is $45 million. Please review all attachments to this notice for details. SCOPE OF WORK: See Section C of the attached solicitation for details. LOCATION OF PROJECT: Fort Thompson, SD CONSTRUCTION DURATION: Period of Performance is 180 calendar days after issuance of a Notice to Proceed. CONSTRUCTION MAGNITUDE: In accordance with FAR 36.204, the magnitude of this construction project is anticipated to be between $100,000.00 and $250,000.00. SITE VISIT: See Section L.1. FAR 52.236-27 SITE VISIT (CONSTRUCTION)(FEB 1995) ALTERNATE I (FEB 1995) CONTRACT TYPE: The IHS intends to award a Firm Fixed Price (FFP) construction contract in support of this requirement. This solicitation will be procured as a Request for Proposal (RFP) in accordance with FAR Part 13 and 36 procedures. QUOTE REQUIREMENTS: See Sections L&M of the solicitation. QUOTE DUE DATE: The quoter’s proposal shall be submitted electronically no later than the due date and time indicated in block 13 of the SF 1442, unless changed by amendment. EVALUATION: In accordance with FAR 13.106-2, the evaluation for this award will be based on Price, Technical Experience Narratives Similar in Scope within the past 5 years and the quoter’s compliance with the terms and conditions for this request for quotes; see Section L. Prices quoted must reflect the quoter’s understanding of the requirement, ability to perform, and be considered fair and reasonable to the Government. Price is the discriminating factor for award among eligible quoters.
