The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2024-05 dated 05/22/2024. This solicitation is set-aside for SBA Veteran Small Business Certification (VetCert) certified and registered Service-Disabled Veteran Owned Small Business (SDVOSB). The associated North American Industrial Classification System (NAICS) code for this procurement is 621111 Offices of Physicians (except Mental Health Specialists), with a small business size standard of $16.0 Million. The FSC/PSC is Q201, Medical-Managed Healthcare. The Department of Veterans Affairs, St Cloud MN VA Hospital, 4801 Veterans Drive, St Cloud MN 56303, is seeking a contractor to provide On-Site Medical Officer of the Day Service (Physician, Physician Assistant, and/or Nurse Practitioner). The anticipated period of performance is 12/1/2024 to 11/30/2025 and includes four (4), one-year option periods. See attached Standard Form 1449, Solicitation Document for Commercial Services. Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Products and Commercial Services. If paragraph (j) of the provision is applicable, a written submission is required. At the time of submission of offer, offerors shall have an active System for Award Management (SAM) registration and SBA Veteran Small Business Certification (VetCert) as a certified and registered Service-Disabled Veteran Owned Small Business (SDVOSB). Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government.
