**Transaction Number:** 678-26-3-071-0175 2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The Urology and Radiology department needs Endocavity biplane Transducer probes (E14CL4B -Catalog 9048) and corresponding reprocessing trays (for E14CL4B and E10C4) to continue providing high quality transrectal and transvaginal ultrasound examinations. The equipment functions as a visualization tool for endocavity tissues and assists in identifying abnormalities in the surrounding area.

The staff will connect the probes to the existing BK3000 system which will enable diagnostic output to assist with real-time guidance for urologic, gynecological and pelvic procedures. Purchasing this equipment will allow clinicians to continue performing necessary diagnostic evaluations and minimally invasive procedures without interruptions, while providing interventional solutions.

**Total:** $78,302.00 3. Unique characteristics or circumstances that limit the requirement to Brand Name Only commodities. This VA facility currently uses the BK3000 Ultrasound system to assist in performing transrectal and transvaginal imaging. The highly specialized system requires compatible transducers to produce diagnostic quality images and perform specialized procedures.

The probes integrate with the system s hardware, software, imaging modes and clinical applications to provide high-resolution ultrasound imaging required for the guided procedures. Alternate manufacturer transducers cannot be utilized with the existing BK3000 system. Purchasing non-compatible or alternate brand probes could result in reduced image quality and loss of functionality.

Manufacturer warranty also prohibits the use of alternate brand name probes/generic alternatives as both pieces of equipment have been FDA cleared as a combined system. The existing set and requested probes are peculiar to one manufacturer and necessary to purchase to ensure continued delivery of safe and effective patient care.

Description of market research conducted and results or statements why it was not conducted: Market research was conducted using DSBS, Adds Up, FPDS, SCMC, and internet websites. A search of SCMC did not render any mandatory contracts for the items requested.

After further market research it was revealed that the required biplane transducer probes and corresponding Page 2 of 2 reprocessing trays are only available from BK Medical /GE Healthcare and one single source SDVOSB distributor. Frontline Healthcare LLC. has been identified as the single source distributor for the manufacturer; BK Medical/GE Healthcare. A search of DSBS listed Frontline Healthcare LLC as a SDVOSB vendor. The vendor is listed in SAM.gov and is not listed on the Expired parties List.
