The Department of Veterans Affairs, Network Contracting Office (NCO) 21, is conducting market research to identify potential vendors with economic status of Service-Disabled Veteran-Owned, Veteran-Owned, and other Small Businesses that can provide the general requirements below. GENERAL REQUIREMENTS Contractor will provide residential care including care for Acts of Daily Living (ADL) and Independent Acts of Daily Living (IADL) care for Veterans and must have an Adult and Senior Care Facility license (Residential Care Facility for the Elderly RCFE ) in the State of CA to provide ADL and IADL care. The program will also provide case management services to homeless Veterans in order to assist them to improve their income, health care, and access to long term permanent housing. The program residential facility must be located in Contra Costa, Solano, Yolo, Sacramento and West Placer Counties in California (See PWS Addendum 1 Zip Codes). The program will provide each Veteran room & board, private or shared bathroom and common area. This contract will be for between 1 and 5 beds, starting with 4 beds, for men and women.
