Please respond to this sources sought notice by sending your reply to both the primary and secondary points of contact (POCs), with a brief capabilities statement and your company's Unique Entity Identification (UEI) number. Interested entities are encouraged to identify any notable barriers to competition and provide any recommendations regarding the DRAFT Performance Work Statement (PWS). Scope of requirement: The Contractor shall be responsible for providing Home Oxygen Services to all beneficiaries serviced by VISN 20 within the State of Alaska ONLY. The contractor shall provide all supplies, materials, equipment, transportation of equipment, equipment services, labor, supervision, patient education, safety management, infection control, as necessary for patients on home oxygen therapy, as specified within the attached Performance Work Statement (PWS). These services shall be required twenty-four (24) hours a day, seven (7) days a week, including holidays. The contractor shall be staffed and stocked to render services provided in accordance with all terms and conditions of this SOW for the duration of the performance period. Interested parties should thoroughly review the specifications and be familiar with the area of coverage before confirming their interest in order to be fully aware of the supplies and services required.
