\n\n\n\n1. Purpose
\n\n1.1 Provide Courier Services for the Rocky Mountain Regional VA Medical Center and the southern Colorado Community Based Care Clinics (CBOCs).
\n\n1.2 The Contractor shall furnish all necessary labor, supplies, equipment, transportation, insurance, vehicle fuel, uniforms, name tags, and/or badges for performance under this contract, to provide Courier Services Monday - Friday, excluding Federal Holidays, in accordance with the specifications and requirements herein.
\n\n2. General Requirements
\n\n2.1 The contractor agrees to provide regularly scheduled pickups for sterile and non-sterile Reusable Medical Equipment (RME).
\n\n2.2 As a Non-Personal Services contract, contractor personnel are not considered VA employees for any purpose. The contractor retains all control over personnel and responsibility for direct supervision.
\n\n2.3 The contractor shall provide a fixed rate for all costs of the courier service, inclusive of vehicle, driver, mileage, time, weight, and other reasonable costs associated with courier services described in this contract.
\n\n2.4 Deliveries will fit easily into a small transport van or utility vehicle.
\n\n2.5 Contractor shall receive items from the southern CBOCs, sign receipt thereof, and transport and deliver the items in the same condition marked by the VA and CBOC.
\n\n2.6 Prior to first delivery, the contractor shall provide evidence to the Contracting Officer (CO) and the COR for each of the following:
\n\n2.7 Contractor Staff Identification and Security Requirements. All contractor personnel providing services under this contract and requiring access to VA premises at the Rocky Mountain Regional VAMC, Pueblo CBOC, or Colorado Springs CBOC shall obtain and display a Department of Veterans Affairs issued Personal Identity Verification (PIV) badge at all times while on VA property.
\n\n3. Scope of Work
\n\n3.1 The contractor shall maintain minimum/maximum temperature and humidity records with the monitor provided by the government to ensure compliance within parameters. Documentation will include:
\n\n3.2 The contractor shall maintain documentation for daily cleaning of the vehicle, including:
\n\n3.3 The contractor shall maintain a record log of courier pick-ups and deliveries to include:
\n\n3.4 The Service Contract Labor Standards shall be applied to this contract in accordance with the McNamara-O'Hara Service Contract Act (SCA).
\n\n4. Specialized Requirements
\n\n4.1 The contractor shall ensure no transportation of illegal or controlled substances and that the driver is not under the influence of alcohol or illegal drugs.
\n\n4.2 The contractor shall develop and maintain a quality control program ensuring the services performed are in accordance with the specifications of the contract.

\n\n\n\nVolume I - Technical capability or quality to meet the Government requirement.
\n\nThe offeror shall submit its quote demonstrating how it meets the requirements of the performance work statement (PWS) and solicitation, to include the quality control plan (see in PWS 4.2). Offeror shall provide all documents as requested in section 2.6 of the PWS.
\n\n\n\nVolume II – Price
\n\nPrice - The offeror shall complete the pricing schedule provided in Section (v) above: Price/Cost Schedule. The offeror shall price the base and all option year line items.
\n\n\n\nVolume III – Past Performance
\n\nOfferors shall provide a maximum of three (3) recent and relevant past efforts (as defined below) to demonstrate their ability to perform the proposed work. If a completed Contractor Performance Assessment Reporting System (CPARS) evaluation is available, it shall be submitted. If not providing CPARS evaluation past performance references must include contract numbers, amount of contract award, point of contact with email address, telephone numbers and other relevant information, and a description of product/work performed under the contract. To be relevant, the effort must be similar in nature of work, size, magnitude, complexity, and scope of this procurement. To be recent past performance is defined as not more than five years from the RFQ release date of this solicitation.

Factor II. Price: The Government will evaluate the price by adding the total of all line-item prices, including all options. The Government may use various price analysis techniques and procedures to make a price reasonableness determination. Offers that do not meet or exceed the technical capability or quality of the service offered to the Government requirement, as stated in Factor 1 shall not be selected regardless of price.
\n\n\n\nFactor III. Past Performance: The Government will evaluate on how well the offeror identified performance relates to the requirement procured under the PWS for this procurement to determine relevancy. How well the offeror performed on identified contracts shall be used to assess performance confidence.
\n\n\n\nIn the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance. The Government may use other methods to evaluate past performance, including but not limited to review of Contractor Performance Assessment Reporting System (CPARS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers, Contracting Officers, and Fee Determining Officials; and as appropriate, the Defense Contract Management Agency (DCMA) Acquisition Information Repository, accessible through the Defense Technical Information Center website.

\n\nThe Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to the prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 45 days.
\n\n52.217-9 Option to Extend the Term of the Contract (MAR 2000)
\n\n(a) The Government may extend the term of this contract by written notice to the Contractor within 45 days provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 50 days before the contract expires. The preliminary notice does not commit the Government to an extension.
\n\n(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
\n\n(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years.

\n\nAddressing DEI Discrimination by Federal Contractors (Deviation DATE)
\n\n(a) Definitions. As used in this clause—
\n\nProgram participation means membership or participation in, or access or admission to: training, mentoring, or leadership development programs; educational opportunities; clubs; associations; or similar opportunities that are sponsored or established by the contractor or subcontractor.
\n\nRacially discriminatory diversity, equity, and inclusion (DEI) activities means disparate treatment based on race or ethnicity in the recruitment, employment (e.g., hiring, promotions), contracting (e.g., vendor agreements), program participation, or allocation or deployment of an entity's resources.
\n\n(b) In connection with the performance of work under this contract, the Contractor agrees as follows:
\n\n(1) The Contractor will not engage in any racially discriminatory DEI activities;
\n\n(2) The Contractor will furnish all information and reports, including providing access to books, records, and accounts, as required by the Contracting Officer, for purposes of ascertaining compliance with this clause;
\n\n(3) In the event of the Contractor's or a subcontractor's noncompliance with this clause, this contract may be canceled, terminated, or suspended in whole or in part, and the Contractor or subcontractor may be declared ineligible for further Government contracts;
\n\n(4) The Contractor will report any subcontractor's known or reasonably knowable conduct that may violate this clause to the Contracting Officer and take any appropriate remedial actions directed by the Contracting Officer; and
\n\n(5) The Contractor will inform the Contracting Officer if a subcontractor sues the Contractor and the suit puts at issue, in any way, the validity of this clause.
\n\n(6) The Contractor recognizes that compliance with the requirements of this clause are material to the Government's payment decisions for purposes of 31 U.S.C. 3729(b)(4).
\n\n(c) The Contractor must include the substance of this clause, including this paragraph (c), in subcontracts at any tier, including those for commercial products and commercial services, except those where the place of delivery or performance is outside the United States.

The offeror shall confine submissions to essential matters, sufficient to define the quote and provide an adequate basis for evaluation. \n\n\n\n1. Purpose
\n\n1.1 Provide Courier Services for the Rocky Mountain Regional VA Medical Center and the southern Colorado Community Based Care Clinics (CBOCs).
\n\n1.2 The Contractor shall furnish all necessary labor, supplies, equipment, transportation, insurance, vehicle fuel, uniforms, name tags, and/or badges for performance under this contract, to provide Courier Services Monday - Friday, excluding Federal Holidays, in accordance with the specifications and requirements herein.
\n\n2. General Requirements
\n\n2.1 The contractor agrees to provide regularly scheduled pickups for sterile and non-sterile Reusable Medical Equipment (RME).
\n\n2.2 As a Non-Personal Services contract, contractor personnel are not considered VA employees for any purpose. The contractor retains all control over personnel and responsibility for direct supervision.
\n\n2.3 The contractor shall provide a fixed rate for all costs of the courier service, inclusive of vehicle, driver, mileage, time, weight, and other reasonable costs associated with courier services described in this contract.
\n\n2.4 Deliveries will fit easily into a small transport van or utility vehicle.
\n\n2.5 Contractor shall receive items from the southern CBOCs, sign receipt thereof, and transport and deliver the items in the same condition marked by the VA and CBOC.
\n\n2.6 Prior to first delivery, the contractor shall provide evidence to the Contracting Officer (CO) and the COR for each of the following:
\n\n2.7 Contractor Staff Identification and Security Requirements. All contractor personnel providing services under this contract and requiring access to VA premises at the Rocky Mountain Regional VAMC, Pueblo CBOC, or Colorado Springs CBOC shall obtain and display a Department of Veterans Affairs issued Personal Identity Verification (PIV) badge at all times while on VA property.
\n\n3. Scope of Work
\n\n3.1 The contractor shall maintain minimum/maximum temperature and humidity records with the monitor provided by the government to ensure compliance within parameters. Documentation will include:

\n\nThe offeror shall submit its quote demonstrating how it meets the requirements of the performance work statement (PWS) and solicitation, to include the quality control plan (see in PWS 4.2). Offeror shall provide all documents as requested in section 2.6 of the PWS.
\n\n\n\nVolume II – Price
\n\nPrice - The offeror shall complete the pricing schedule provided in Section (v) above: Price/Cost Schedule. The offeror shall price the base and all option year line items.
\n\n\n\nVolume III – Past Performance
\n\nOfferors shall provide a maximum of three (3) recent and relevant past efforts (as defined below) to demonstrate their ability to perform the proposed work. If a completed Contractor Performance Assessment Reporting System (CPARS) evaluation is available, it shall be submitted. If not providing CPARS evaluation past performance references must include contract numbers, amount of contract award, point of contact with email address, telephone numbers and other relevant information, and a description of product/work performed under the contract. To be relevant, the effort must be similar in nature of work, size, magnitude, complexity, and scope of this procurement. To be recent past performance is defined as not more than five years from the RFQ release date of this solicitation.

Factor II. Price: The Government will evaluate the price by adding the total of all line-item prices, including all options. The Government may use various price analysis techniques and procedures to make a price reasonableness determination. Offers that do not meet or exceed the technical capability or quality of the service offered to the Government requirement, as stated in Factor 1 shall not be selected regardless of price.
\n\n\n\nFactor III. Past Performance: The Government will evaluate on how well the offeror identified performance relates to the requirement procured under the PWS for this procurement to determine relevancy. How well the offeror performed on identified contracts shall be used to assess performance confidence.
\n\n\n\nIn the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance. The Government may use other methods to evaluate past performance, including but not limited to review of Contractor Performance Assessment Reporting System (CPARS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers, Contracting Officers, and Fee Determining Officials; and as appropriate, the Defense Contract Management Agency (DCMA) Acquisition Information Repository, accessible through the Defense Technical Information Center website.

\n\nThe Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to the prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 45 days.
\n\n52.217-9 Option to Extend the Term of the Contract (MAR 2000)
\n\n(a) The Government may extend the term of this contract by written notice to the Contractor within 45 days provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 50 days before the contract expires. The preliminary notice does not commit the Government to an extension.
\n\n(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
\n\n(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years.

\n\nAddressing DEI Discrimination by Federal Contractors (Deviation DATE)
\n\n(a) Definitions. As used in this clause—
\n\nProgram participation means membership or participation in, or access or admission to: training, mentoring, or leadership development programs; educational opportunities; clubs; associations; or similar opportunities that are sponsored or established by the contractor or subcontractor.
\n\nRacially discriminatory diversity, equity, and inclusion (DEI) activities means disparate treatment based on race or ethnicity in the recruitment, employment (e.g., hiring, promotions), contracting (e.g., vendor agreements), program participation, or allocation or deployment of an entity's resources.
\n\n(b) In connection with the performance of work under this contract, the Contractor agrees as follows:
\n\n(1) The Contractor will not engage in any racially discriminatory DEI activities;
\n\n(2) The Contractor will furnish all information and reports, including providing access to books, records, and accounts, as required by the Contracting Officer, for purposes of ascertaining compliance with this clause;
\n\n(3) In the event of the Contractor's or a subcontractor's noncompliance with this clause, this contract may be canceled, terminated, or suspended in whole or in part, and the Contractor or subcontractor may be declared ineligible for further Government contracts;
\n\n(4) The Contractor will report any subcontractor's known or reasonably knowable conduct that may violate this clause to the Contracting Officer and take any appropriate remedial actions directed by the Contracting Officer; and
\n\n(5) The Contractor will inform the Contracting Officer if a subcontractor sues the Contractor and the suit puts at issue, in any way, the validity of this clause.
\n\n(6) The Contractor recognizes that compliance with the requirements of this clause are material to the Government's payment decisions for purposes of 31 U.S.C. 3729(b)(4).
\n\n(c) The Contractor must include the substance of this clause, including this paragraph (c), in subcontracts at any tier, including those for commercial products and commercial services, except those where the place of delivery or performance is outside the United States.
