This conference will educate a multi-professional audience on advancing care for older adults with serious illnesses. It will focus on the unique challenges and needs of geriatric patients facing hospitalization and surgery. Topics will include aligning surgery with patients goals, managing geriatric specific health needs, improving discharge processes, and fostering interdisciplinary collaboration in care. Additionally, the conference will address key policy issues, such as age-friendly measures, state aging master plans, and advocacy efforts, while emphasizing the importance of shared decision-making and co-management. By offering relevant educational content, the event aims to promote best practices, support professional development, and provide Continuing Medical Education (CME) credits for participants. The conference is being sponsored by the University of Utah, Division of Geriatrics and the Geriatric Research, Education and Clinical Center (GRECC) on September 11-12, 2025. The University is incurring all expenses and the GRECC Service will reimburse the University for a portion of the costs, based on the Memorandum of Understanding (MOU) between VISN 19 and George E.
