Scope of work: The Contractor s technical personnel shall be certified and trained in ESWL services and operations of the unit(s). The Technician will assist the attending Urologist in the operation of the device. The technician may be required to operate the device and any related imaging equipment during procedures. Prospect contractor s technicians(s) shall have a current license to perform services in the state of Utah when services are performed. **Place of Performance:** **Place of Performance:** Contractor shall provide on-site services at VASLCHS 50.0 Foothill Blvd. Salt Lake City, Utah, 84148.
