The NAICS Code for this request for quotes is 334516 (1,000 Employees). A total small business set-aside is used for this solicitation. The items being requested are as follows: Two fully automated Erythrocyte Sedimentation Rate (ESR) instruments that will include onsite service and maintenance, reagents, controls, calibrators, and supplies using the Cost Per Test (CPT) invoicing method for estimated quantities of 4,440 test annually. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The evaluation criteria are technical acceptability and price. Under technical includes the ability to provide all of the requested items in accordance with the Statement of Work, provide a letter of authorization to distribute and provide maintenance services (if not the manufacturer). Offerors are to provide a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer.
