## Description

The purpose of this requirement is to initiate a new service agreement/contract for the Mizuho Hana and Trios surgical tables that are currently being used in the facility. The proposed service agreement/contract would cover all necessary usage maintenance, preventive maintenance, parts and labor for a base year of twelve (12) months and four (4) option years that are 12 (twelve) months in length (for each option year).
