## Description

Contractor shall provide preexisting software, hardware (if applicable), media, software license, software maintenance/upgrade, installation, training, and technical support services for the Patient Checkout Discharge Instruction System, as well as the system server to the following facilities: Clement J. Middleton Memorial Veterans Hospital located at 2500 Overlook Terrace, Madison, WI 53705 607-25-1-053-0001/607-3650160-053-824100-2324 010024127 Edward Hines, Jr.

VA Hospital located at 5000 South 5th Avenue, Hines, IL 60141 578-25-1-022-0001/578-3650160-022-820100-2560 010020194 The software shall enable the timely and correct use of technology for patient discharge summary instructions in the Emergency Department at each facility. The contractor software shall also include content for patient education, discharge documents, and other miscellaneous documents related to patient education.

This requested Education Software will continue to aid physicians and nurse practitioners in providing quality care to the VA Veterans by enabling the timely and correct use of technology for patient discharge summary instructions.
