The Louis Stokes Cleveland VA Medical Center is seeking sources for a multi-year contract (Base Year plus One Year Option) is seeking contractors to provide coding assignments for the International Classification of Diseases (ICD) for Diseases and Procedures, Common Procedural Terminology and Healthcare Common Procedural Coding System Level II codes based on health record documentation of outpatient, surgical, and inpatient episodes of care provided by joining the National Coding BPA. . The vendor must also provide a service and maintenance agreement which will include making adjustments, repairs, and replacing any malfunctioning equipment. The agreement will also include answering to service calls and providing startup and training for the VA staff. Response to this notice should include a capability statement, and three (3) customer references to which you have provided similar services for. Responses should also include company name, address, point of contact, size of business pursuant to the following questions: (1) Is this product or service available under a federal supply schedule (GSA)? If yes, please provide the contract number. (2) Do you manufacture or directly provide these services? If teaming please provide a copy of your teaming agreement.
