Company name, address, point of contact, and point of contact email address and phone number. 2. UEI number and CAGE code. 3. Business size status under the applicable NAICS code. 4. Existing government Contract Number/ Contract for this scope and expiration date. 5. What percentage of the total contract value will be subcontracted? 6. Identify what portions of the work you would perform using in house staff versus subcontractors.
