VISN 04 CPRR Lab Instrumentation Urinalysis Chemistry Solicitation# 36C24425Q0739 Vendor Questions / Government Responses Per Attachment A Instrument Requirements, every location, except for Coatesville VAMC, is requiring a primary analyzer and a backup analyzer With Macroscopic and Microscopic capabilities- is that correct?Â The backup instrument requires the same functionality as the primary instrument?Â Back up analyzers are macroscopic only.

Also, page 29 h.) Primary and backup analyzers (for larger facilities) shall be from the same manufacturer and offer the same technology- please define larger facilities ?Â Large facilities would be facilities testing above 15,000 macroscopic per year. Large facilities: Erie, Lebanon, Philadelphia, Pittsburgh, Wilkes-Barre and Wilmington. In addition, please clarify what the backup requirement is for each location? All backups are all fully- automated Macro Only.

If each site has a different requirement for their backup can you please clarify what each site requires for a backup? Such as, fully automated Macroscopic and Microscopic, fully automated Macro only, or Semi-Automated Macro only All backups are all fully-automated Macro Only. Can you please confirm if any of the labs will require a wagon/cart that the instrument would sit on?Â Or will all instruments be sitting on existing countertop or table space? No space is available for a wagon/cart.

Analyzer/equipment must fit within measurements provided. In the Statement of Work Attachment D Past Performance Questionnaire- can you please confirm, you would like this document to be filled out by the reference and sent to you directly?Â Please confirm you are looking for one (1) reference?Â I don t see a number of references required.Â The Past Performance Questionnaire (PPQ) is completed by the Reference evaluating past performance and returned directly to email address in the heading of the PPQ. A minimum of (3) PPQs may be submitted. (return to:

stanley.kwasny@va.gov). Would submission of CPARS be an adequate replacement for Attachment D? CPARs can be used as Past Performance. We noticed in Urinalysis SOW - Attachments (Attachment A Instrument Requirements) that Butler is showing requirements for backup analyzer. Please confirm whether to include a backup for Butler. Thank you! Butler VAMC does not require a Backup Analyzer. Do not include a backup for Butler VAMC.

Additionally, the label of Column D appears to be incorrect, Back Up Analyzer Macroscopic / Microscopic we believe should read Back Up Analyzer Macroscopic Only . In Attachment A - Column D has been changed to Back up Analyzer Macroscopic During Open Market Research, we believe that only Chemistry Macroscopic backup testing is required. Can you please provide confirmation? Thank you! All backups are all fully- automated Macroscopic Only.

I wanted to clarify the Urinalysis requirement for VISN 4 posted on SAM.gov- is this an FSS solicitation or Open Market?Â The pre-solicitation notice expressed this would be Open Market but the actual solicitation references the FSS terms. This Solicitation is Open Market and we are accepting open market submissions. Any mention of FSS in the RFQ was an error and an amendment will be posted to Sam.gov to correct any areas.
