Any disruption to the facility shall be coordinated through the Contracting Officer s Representative (COR) and Contracting Officer. Suitable line voltage electric will be supplied by CVAMC personnel. Contractors are to be fully briefed on the importance of tool safety where applicable to patient care. Contractor shall provide all supervision, necessary labor, tools, equipment, permits, testing and reports in accordance with all federal, state and local codes and regulations and materials to perform the job. All debris shall be removed from site and recycled along with tickets and weights. Any construction damage incurred because of installation is the responsibility of the contractor. If at any time during the construction something is uncovered that poses a risk to safety or function in any facet of the CVAMC, the contractor shall cease production and notify the COR and shall not proceed until authorized in writing by the Contracting Officer. Scope of Work: Form pit and 12-inch-thick protective wall encasement with steel curb angle iron with four 3 PVC drain holes and with 1/2-inch rebar cage. Install dual spotlight and switch to cover dock and inside trailer. This wall is 12 thick reinforced concrete per drawing to include dock bumpers and steel corner angles. Bottom slab pitched towards front. This area must contain a reinforced concrete pad capable of 6000 pounds. This is the pit area which must have 12 thick walls, a trough for electrical and hydraulics in accordance with lift manufacturer requirements. The pad must be pitched down toward the front at minimum 1/4 inch per foot. Extend downspout ground leader above new dock, and tie new downspout into new ground leader This area is filled with cage- reinforced concrete, extending the dock at a 20-degree angle. This area filled with block and reinforced concrete slab Remove board and repair brick in this area with color matched brick. Max. unsupported length 4 ft. during repairs. Patch pavement to match existing after installation of pit and seal at all edges. Raise ground leader to top of new pad and install new copper downspout to match existing in straight line. Clean out exist. leader prior to installation. Pit for the hydraulic scissor lift. North side of pit is to be at approx. 20-degree angle off the building wall. Coordinate with COR. Disconnect and control buttons in lockable weather tight box. Install dual spotlight and switch to cover dock and inside trailer. Install rubber bumpers and imbed angle steel on corners.
