The selected contractor must provide asbestos abatement project monitoring and air sampling/testing services, inclusive of all labor, equipment, supplies, transportation, laboratory analyses and incidentals to perform project monitoring and air sampling/testing of asbestos abatement activities performed by others at VA New York Harbor Healthcare System (NYHHHS) facilities. Projects may be assigned at: Brooklyn VA Medical Center, 800 Poly Place, Brooklyn, NY 11209 Margaret Cochran Corbin (Manhattan) VA Medical Center, 423 East 23rd Street, New York, NY 10010 St. Albans VA Community Living Center, 179-00 Linden Boulevard, Jamaica (Queens), NY 11425 I: Project Description The work to be performed under this Firm Fixed-Price service contract and in accordance with these contract documents includes furnishing all necessary labor, materials, and incidental related work described in the Scope of Work requirements issued under the contract. The work will include monitoring of abatement of asbestos-containing materials and related air sampling/testing. Services are to be provided by an Industrial Hygiene Contractor (IHC) with documented experience in asbestos abatement monitoring and air sampling/testing. All personnel assigned to projects must maintain professional licensing applicable to tasks being performed (e.g., New York State asbestos monitor licensing for oversight of asbestos abatement projects). The IHC shall be prepared to promptly begin work (within ten [10] calendar days) upon authorization to proceed with assigned tasks. Work outside of regular working hours will be required, in accordance with a schedule determined by the VA. Regular working hours are 8:00 AM to 4:30 PM, Monday through Friday, excluding Federal holidays. II: Scope of Work The services ordered under this contract are enumerated and described below. These tasks will include asbestos abatement project monitoring consisting of oversight of abatement activities and air sampling/testing to confirm safe re-occupancy. Task 1 Asbestos Abatement Project Air Monitoring The vendor must provide an experienced and licensed on-site IH or IHT as abatement project monitor to continuously oversee performance of the contractor during abatement work and ensure adherence to applicable Federal and state asbestos abatement laws and standards. The on-site monitor must have a minimum of two years of experience in the field. The monitor must perform a pre-abatement containment inspection including a thorough review of all conditions including but not limited to negative pressure filtration equipment, air locks, and signage. The vendor must regularly monitor the integrity of negative pressure containment during the abatement process including performing smoke tests and taking manometer readings as required and collect samples to document air quality in and around the work area prior to, during, and following the completion of each abatement. Document and oversee the efficacy of work procedures, air filtration systems, the abatement process, and the wetting, bagging, moving, and loading of the asbestos-containing material removed. If at any time the IHC suspects or establishes a release of asbestos outside of the work area and/or identifies regulatory noncompliance by the abatement contractor, the IHC must direct the abatement contractor to stop work and immediately notify the VA. Task 2 Asbestos Final Air Clearance Monitoring Following ACBM removal and cleaning of the containment area by the abatement contractor, the vendor must inspect the workspace and ensure that all visible asbestos has been removed in accordance with all applicable regulations. Ensure that the abatement contractor performs proper lockdown procedures in accordance with applicable regulations. Performs the required final air testing to determine whether the safe re-occupancy asbestos fiber level mandated by regulatory requirements has been met. Secure the services of a qualified analytical laboratory to turn over sample results within a maximum of 8 hours unless a specific alternative request has been accepted by the VA. Task 3 Asbestos Abatement Compliance Report The vendor must provide a certificate to the VA certifying that the abatement process was completed in accordance with all applicable regulations and the project specifications. The vendor must provide a final compliance report including regulatory notifications; contractor submittals including worker licenses and training certifications, project monitor s daily logs; sign-in and sign-out sheets; sketch or drawing clearly identifying abatement area and defining the type of extent and quantity of ACBM abated and documenting any known ACBM left behind in the work area; any project photographs; project air monitoring data; and contractor waste disposal manifests. If the IHC is unable to obtain the waste disposal manifests from the abatement contractor within 30 days following completion of abatement, the VA must be notified in writing. Asbestos Abatement Sites The vendor must perform the aforementioned services for the following abatement projects: Brooklyn VA Medical Center: Building 1, 2nd Floor, Rooms 2-105, 2-105A, 2-105B, 2-105C, and 2-105D Removal and disposal of a total of 980 SF of carpet, asbestos-containing floor tile and asbestos-containing mastic. Building 1, 3rd Floor, Room 3-301 (Histology Laboratory) Removal and disposal of five (5) individual laboratory islands within room 3-301. All utility disconnection (plumbing, power etc.) to be performed by VA. All stored items to be removed by VA. Each island has a cementitious asbestos-containing benchtop. The total quantity of asbestos benchtop material is approximately 250 square feet. Contractor is responsible for the removal and disposal of the ACM cementitious benchtop as well as the non-ACM base portions of the laboratory islands. Building 1, 8th Floor, Room 8-105 -- Remove 20 SF of asbestos-containing floor tile and asbestos-containing mastic. Building 1, 9th Floor, Room 9-107 Remove 70 SF of asbestos-containing floor tile and asbestos-containing mastic. Margaret Cochran Corbin (Manhattan) VA Medical Center: Building 1, 7th Floor, Room 7036C Removal of approximately 150 square feet of asbestos-containing floor tiles, mastic, and cove base adhesive in an office. Building 1, 10th Floor, Rooms 10058S, 10073S, 10085S, 10087S Removal of approximately 30 SF of non-asbestos CMU block to access 35 LF of ACM pipe insulation for removal in each of four plumbing chases. Total quantities for this area are 120SF of non-ACM CMU block and 140 LF of ACM pipe insulation. Building 1, 16th Floor, North Hallway, and Rooms, 16140N, and 16142N Removal of approximately 1,074 SF of asbestos-containing floor tiles, mastic, and cove base adhesive. Building 1, 18th Floor, Rooms 18047S, 18049S, 18093S, 18095S, 18097S, and 18098S Removal of a total of approximately 1,015 square feet of carpeting, asbestos-containing floor tiles, mastic, and cove base adhesive in six (6) separate offices (each office ranges from 115 SF 300 SF). Building 2, Rooms 105A, 108A, 116A, 129A, 132A, 147A, and 148A Removal of a total of approximately 1,320 SF of carpeting, asbestos-containing floor tiles, mastic, and cove base adhesive in seven (7) separate offices (each office ranges from 125 SF 250 SF) St. Albans VA Community Living Center: Building 64 (Boiler Plant), Basement Incinerator Room -- Properly containerize approximately one (1) cubic yard of asbestos waste and asbestos contaminated debris located in the boiler plant incinerator room. Establish critical barriers/containment and HEPA vacuum area. Building 64 (Boiler Plant), 1st Floor, Main Office -- Remove approximately 250 square feet of asbestos-containing floor tile and mastic. Building 64 (Boiler Plant), Catwalk -- Remove approximately 30 linear feet (LF) of asbestos pipe insulation accessible from a catwalk in the plant. Building 87 (Chapel Area), Rooms B108 - B110 Pipe Chase 1: Remove 20 linear feet of asbestos-containing pipe insulation and 20 SF of asbestos-contaminated plaster to access and abate the ACM pipe insulation. Pipe Chase 2: Remove 20 linear feet of asbestos-containing pipe insulation and 20 SF of asbestos-contaminated plaster to access and abate the ACM pipe insulation. Building 89 (Home-based Primary Care), Rooms A325, A327, and A329 Pipe Chase 1: Remove 25 linear feet of asbestos-containing pipe insulation and 25 SF of asbestos-contaminated plaster to access and abate the ACM pipe insulation. Pipe Chase 2: Remove 25 linear feet of asbestos-containing pipe insulation and 25 SF of asbestos-contaminated plaster to access and abate the ACM pipe insulation. Building 89 (Quality Management Office), Rooms A302 A307 Remove approximately 700 square feet of carpeting, asbestos-containing floor tile and mastic. Building 89 & 91 (steam pipes along the basement corridor) Remove 150 linear feet of asbestos-containing pipe insulation. Building 91, Room E215 -- Remove approximately 40 SF of asbestos-contaminated wall plaster; access and remove approximately 25 LF of asbestos pipe insulation in the wall chase. Remove an additional 25 LF of exposed asbestos pipe insulation along the ceiling of the same room. Project Schedule and Coordination The vendor will commence asbestos abatement project monitoring and air sampling/testing activities coinciding with the start of asbestos abatement activities. The vendor will complete all services, including reporting within seven (7) calendar days after post-abatement clearance sampling has taken place. The vendor will designate a project manager who will serve as the main point of contact for all communication, scheduling, and reporting for the duration of this project. Regular coordination meetings must be held with the abatement contractor and VA personnel to ensure seamless integration of abatement and air monitoring activities. III: Regulatory Compliance All asbestos abatement project monitoring and air sampling/testing activities will be performed in strict compliance with the following regulations: U.S. Environmental Protection Agency (EPA) Asbestos Hazard Emergency Response Act (AHERA). Occupational Safety and Health Administration (OSHA) Asbestos Standards (29 CFR 1926.1101). National Emissions Standards for Hazardous Air Pollutants (NESHAP) regulations. Any relevant New York State asbestos abatement and air quality regulations. IV: Health and Safety The vendor must comply with all applicable health and safety regulations and provide a project-specific health and safety plan to address risks associated with asbestos air monitoring. All personnel conducting air monitoring activities must wear appropriate personal protective equipment (PPE) while in the work area. V: Equipment and Materials The vendor will supply all necessary equipment and materials for asbestos abatement project monitoring and air sampling/testing, including, but not limited to: High-volume air samplers Personal air monitors for workers PCM/TEM analysis supplies PPE for monitoring staff VI: Qualifications Industrial Hygiene Contractor (IHC) The IHC must have a CIH that is certified by the American Board of Industrial Hygiene and in comprehensive practice, a) meet OSHA/EPA's definition of \"Supervisor/Competent Person\" under 29 CFR 1926.1101(o); b) have extensive experience with abatements involving service maintenance personnel; c) have completed formal training in respiratory protection, waste disposal, asbestos abatement/management, and performing visual asbestos assessments/inspections and be accredited as an asbestos Building Inspector/Management Planner; d) have performed as the Senior IH on at least thirty (30) assessment/abatement of projects of similar size and complexity; e) be an EPA/State accredited Project Designer and have participated in the abatement project design of thirty (30) projects of similar size and complexity as a Senior IH member of A/E or VA teams; f) have experience supervising abatement project monitoring/management for at least thirty (30) of similar size and complexity and have all required federal state and local licenses and accreditations. The qualified IHT or IH assigned to perform on-site work shall possess: 1) specialized training and expertise in asbestos abatement management, monitoring, inspection, sampling and analysis on other similar asbestos abatement projects; 2) certificates of training and proficiency in these areas; 3) all federal, state and local licenses and accreditations when required; 4) experience in at least ten (10) projects of similar size and complexity to this project; 5) and experience in performing final clearance inspections and air sampling. Laboratory Laboratories used for asbestos bulk and air sampling analysis must be successful participants in EPA quality assurance for both bulk analysis and the American Industrial Hygiene Association (AIHA) Proficiency Analytical Testing Program. Air samples shall be analyzed by an individual meeting the requirements of OSHA 29 CFR 1926.1101 Appendix A. PCM air samples must be sent to an AIHA laboratory accredited for fiber counting. TEM air clearance samples must be sent to an EPA NIST/NVLAP accredited laboratory for analysis. VII: Fixed Price Compensation The vendor must provide asbestos abatement project monitoring and air sampling/testing services for a fixed price. This price will cover all labor, equipment, materials, analysis/laboratory costs, out-of-pocket expenses, and reporting associated with the project, including pre-abatement air monitoring, daily air monitoring during abatement, post-abatement clearance testing, and reporting and documentation. No additional charges will be incurred unless there are agreed-upon changes to the scope of work. A breakdown of the price per project site and work area must be stipulated in the vendor price quotation. Assumptions and Exclusions The fixed price assumes the following: The length of this contract will end when all the work areas delineated in Section 2: Scope of Work Asbestos Abatement Sites have been abated and corresponding air monitoring activities have been completed and reports have been delivered to VA. Work will be performed outside of regular business hours, and/or at the direction of the VA. Regular business hours are Monday through Friday, 8:00 AM to 4:30 PM. The scope includes additional air sampling or re-clearance testing that may be required due to abatement contractor errors if they are within the limits delineated in these contract documents. VIII: Evaluation Criteria The ranking criteria listed below will be used to evaluate IHC consultants that meet the minimum qualifications criteria. The ranking criteria consider experience, quality, reputation, and other capabilities of an IH/CIH consultant beyond the minimum levels. Proximity of IHC office to project location. Capacity of IHC to support size and complexities of projects delineated in this Statement of Work. Experience of IHC and CIH with asbestos abatement work and VA abatement work. Expertise and experience of Qualified IH as consultant. Competitive unit prices for rates of key persons and lab services. Privacy Training for contractor without access to VA Sensitive Information All contractor employees and subcontractors participating under this contract are required to complete the VA's Privacy training requirement - VA Privacy Training for Personnel without Access to VA Computer Systems or Direct Access to or the Use of VA Sensitive Information. Contractors must provide certifications of completion to the COR during each year of the contract. Training expires 365 days after the training is taken. This requirement is in addition to any other training that may be required of the contractor and subcontractor(s). The COR for the contract is responsible for ensuring the contractor takes the training and to follow up with the annual requirements.
