Special Notice Special Notice Page 3 of 3 Special Notice INTENT TO SOLE SOURCE: Alcon Vision LLC Service and Repair of Constellation LXT Cr5 This is not a request for competitive proposals. This is not a solicitation or request for offers. No solicitation is available. The Department of Veterans Affairs, NCO1 intends to award a One-Time, Firm Fixed Price, Sole Source Service Contract for Service and Repair to the Large Business (LB)

**OEM (Original Equipment Manufacturer), Alcon Vision LLC, UEI:** PHBTLNJ4WCM5, located at 6201 South Fwy, Fort Worth, Texas 76134-2001. The Department of Veterans Affairs Medical Center located at 830 Chalkstone Avenue, Providence RI 02908-4734 has a requirement for a Service and Repair agreement. The Service, Repair and Maintenance is proprietary to Alcon Vision LLC and is the only authorized vendor able to perform this service.

The authority for applying the Simplified Procedures for Commercial Items of FAR 13.5 is 41 U.S.C. 1901. The intent is to procure the referenced services on a sole source basis.
