## Description

NOTICE OF INTENT FOR BRAND NAME PRODUCTS MIDMARK CORPORATION Response Due Date: April 11, 2024, by 10:00AM (CST) Network Contract Office 1, Procurement Office East on behalf of The Department of Veterans Affairs, Northampton VA Healthcare System, located at 421 North Main Street, Leeds, MA 01053 intends to award a firm fixed price contract for Walman Instruments Brand Name products as follows:

The Contractor will provide and install the below listed equipment and ensure that it is functioning as intended. Specific equipment required as listed, quantity of one each: Volk 20 Diopter Large Lens (Clear) (P/N V20C) QTY: 17 Volk 78 Diopter Large Lens (Clear) (P/N V78C) QTY: 17 Volk 90 Diopter Large Lens (Clear) (P/N V90C) QTY: 19 Performance Requirements The contractor s responsibilities will include the following:

Shipping of Equipment Delivery of Equipment Installation of Equipment if required Calibration of Equipment as needed Deliverables The contractor will provide all necessary operator and maintenance manuals The North American Industry Classification System code (NAICS) for this requirement is 339112. All responsible sources that feel they should be considered qualified to provide the above-mentioned products may submit a response to this notice (by the response due date shown above) to sharieka.wright@va.gov .

The information provided shall include detailed (and relevant) documentation to allow the government to perform adequate market research and make a proper determination. A determination by the government not to compete this proposed requirement based upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. THIS IS NOT A REQUEST FOR COMPETITIVE PROPOSALS.
