## Description

An agreement to provide ophthalmology services for needed laser eye exams for required laser eye surveillances if the individual is not able to meet the criteria of Fermilab's medical examination. • Appointments must be accommodated withing 3-5 business days of said authorization • The only billable services authorized is the laser eye exam. CPT code 92004, a refraction fundus photos to be done – initially it would be necessary to have the photos along with the employees best corrected vision in order to evaluate any future concerns • Results must be faxed to Fermilab’s Occupational Medical Office, 630-840-3053, withing 24 hours of exam
